Out in the Cold is the second mission of Call of Duty: Modern Warfare 4, played as Captain Price five days before the invasion; you tail Milena through a nightclub, fight up to the rooftops, chase Makarov and beat him in a hand-to-hand fight, and it unlocks the "No More Rules" Calling Card.
Out in the Cold is the second mission of Call of Duty: Modern Warfare 4, and the first played as Captain Price: it jumps back to five days before North Korea's attack, and Price tails Makarov through a nightclub, over rooftops and through the streets into an alley for a hand-to-hand brawl (Windows Central, Oct 17, 2026). Its four objectives are "Follow Milena," "Locate Makarov," "Chase Makarov" and "Fight Makarov" (TheGamer, Oct 25, 2026), and it is set in New York (Critical Hits, Oct 20, 2026), the city Activision gives for the campaign's close-quarters combat (Call of Duty, read Oct 25, 2026). Finishing it unlocks the "No More Rules" Calling Card (Call of Duty, Oct 16, 2026).

Follow Milena
Price is after Milena Romanova, the Konni Group's financier (Insider Gaming, Oct 20, 2026). Push through the club's crowd and keep her close: falling too far behind fails the mission, and TheGamer says to follow the objective marker if you lose her (TheGamer). The bouncers in the way are melee fights, which teach the block and counter you need later: Critical Hits walks you past one guard, then a second, then two more on the way to the dance floor, and a finisher on the guard who blocks Milena's way to the elevator (Critical Hits). A cutscene shows Makarov stepping into the elevator with her. Beat the two guards at the bar, take the elevator, and beat the two Konni guards inside it to pick up a Krait P68 pistol.
Locate Makarov
When the doors open upstairs the mission turns into a gunfight across the club's upper floor; Kotaku notes that Price fights through the club and its back rooms with only a pistol (Kotaku, Oct 20, 2026). Use the furniture and columns as cover and watch for civilians. Through the entertainment rooms, more enemies come through a service door; in the kitchen corridor an enemy shoves a civilian out from behind a vending machine, and a VEL 46 submachine gun is there for the taking (Critical Hits).
Safety's On trophy (the kitchen)
The kitchen is the Safety's On trophy: "Defeat all enemies in the kitchen of Out in The Cold without firing your gun on Hardened or harder" (Insider Gaming, Oct 19, 2026). PowerPyx's method is to collect four bottles before the kitchen, then throw bottles and use takedowns on the five enemies and melee the last one; a thrown bottle disarms an enemy. You can play the earlier part on an easier setting and switch to Hardened once you reach the corridor (PowerPyx, Oct 18, 2026).
From the kitchen, head to the manager's office, where a cutscene plays and Price deals with Milena; Insider Gaming says he kills her (Insider Gaming). Then fight the Konni guards across the rooftops until you find Makarov waiting for his helicopter.
Chase Makarov
The chase is linear, with no shooting: stay close to Makarov while you jump, slide and climb, press jump out of a slide, and watch for drops that kill you. Falling too far behind sends you back to the last checkpoint (Critical Hits).
Fight Makarov
Makarov is cornered in an alley, and the mission ends in the first of the campaign's melee "boss fights," fought with fists or a knife, where a block timed well parries and opens a counter (GameSpot, Oct 16, 2026). Game Rant breaks it into three parts: a quick-time opening (parry his first strike, counter, parry his spinning stab, pin him against the shutters, then parry to make him drop the knife), an alley fight of hits, parries and a takedown on the melee button, and after a cutscene a last round (Game Rant, Oct 17, 2026). Its two tips: do not land too many hits in a row, because Makarov starts blocking, and hold block just before his strikes land, then attack.
Can't Touch This trophy
Can't Touch This asks you to "Defeat Makarov on Veteran difficulty or higher without getting hit in the alley" (Insider Gaming). Block is L2 or LT, and there is a checkpoint partway through the fight (PowerPyx), so Critical Hits suggests reloading the checkpoint the moment you are hit (Critical Hits).
How it ends
Price pins Makarov and beats him to death: Game Rant lists Makarov's death in this mission as "Beaten to death by Price" (Game Rant, Oct 19, 2026). After the fight Price is captured and taken to Valeria Garza, who offers him a way to take down the Archer, "the head of the snake" (Windows Central). Reward and what comes next
Out in the Cold unlocks the "No More Rules" Calling Card on any difficulty (Call of Duty) and has no collectibles (PowerPyx, Oct 21, 2026). Next is Mission 3: Following Orders, seen from the North Korean side. All missions are on Campaign Walkthrough.
